This phone consultation will allow you to learn more about the care we provide and allow us to determine if you are a candidate for an Initial Examination in our office. If we agree that you would benefit from care in our office, we will offer you an appointment at the end of the consultation.
- Call (828) 322-4787 to schedule an appointment, then download and complete our required office forms PRIOR to your appointment. If you like you can email the forms back to us, and we will print them out for you. If you are unable to download the forms, please call to request that we fax or mail the office forms to you so you can fill them out before your first visit. Please note: by filling out your paperwork prior to your visit, you will reduce your visit in our office by 15-30 minutes.
